Other Comments

The LIFQHC Behavioral Health Services are provided within the framework of integrated health care. The LIFQHC centers strive to follow the Patient Centered Medical Home (PCMH) model where patients are provided with multiple services, including medical and mental health services, under the same roof. This addresses many traditional challenges of mental health service delivery including access to care, mental health stigma, early screening and prevention based models of services and mind-body integration in the provision of treatment. Integrated health care ensures that patients’ needs are met in a comprehensive manner through coordination of care between multiple service providers. This results in better outcomes for patients and cost-benefits for institutions through on-going treatment within local facilities, rather than crisis-based treatment at inpatient facilities. At LIFQHC, there are both ad-hoc and regularly scheduled interactions between the behavioral health team and the primary care and other specialty teams, including internal medicine, pediatrics, OB/GYN, dental, podiatry, etc. which ensures a high degree of integrated health care for our patients.
