Dorothy B. Hersh Child Protection Center at Saint Peter's University Hospital

Provide description of Externship:

The Dorothy B. Hersh Child Protection Center (CPC) is a child maltreatment program associated with Saint Peter’s University Hospital, located in Middlesex County, New Jersey. The Dorothy B. Hersh CPC provides children and families with psychological and medical evaluations related to child abuse concerns that include medical, emotional, physical, and sexual abuse. The staff consists of a multidisciplinary team of health care professionals including pediatricians, registered nurses, psychologists, social workers, and pediatricians. The program utilizes a team approach that includes multidisciplinary case conferences and collaboration with community agencies such as child protection agencies and law enforcement. The Dorothy B. Hersh CPC is housed in the St. Peter’s Family Health Center which offers a range of behavioral and medical services to both pediatric and adult populations. Many opportunities for further learning are available via supervision, weekly didactic presentations, in-services, and pediatric rounds. The Dorothy B. Hersh CPC is an APA Internship grant recipient and a member of the APPIC match program. Interns and Externs participate in training and educational seminars, as well as supervision with experienced psychologists, to enhance their skills related to the evaluation and treatment of children, adolescents, and adults. Various treatment techniques are utilized, including Trauma-Focused Cognitive Behavioral Therapy (TF-CBT), Play Therapy, and Attachment Theory. Additional training is provided in Game-Based Group TF-CBT, an integration of play therapy and TF-CBT. Both Interns and Externs will have the opportunity to co-facilitate groups utilizing this innovative evidence-based approach. Intern and Extern duties include: Psychosocial & Psychological Evaluations; Psychological Testing; and Group Therapy. Populations served are widely culturally and socio-economically diverse. Current supervisors represent a range of cultural backgrounds and theoretical orientations. Internship requires 12 months of training, over a 40-hour work week. Externships require 10 months of training, over a minimum 18 hour work week.

Intervention and Assessment:

Intervention training experiences offered: Estimate average number of individual patients that students will carry; estimate number of groups and other types of interventions. Offer description: Externs are asked to meet with four therapy cases each week and they conduct one evaluation.
Assessment training experiences commonly offered. Describe: Training is provided in conducting forensic screening evaluations for children who have recently been removed from their primary caregivers.  Externs are also trained in conducting in depth psychological evaluations of children and their caregivers, where child maltreatment has been suspected or confirmed.  Standardized self-report and administered assessments are part of the evaluations.

Context of didactics (seminars, Grand Rounds): Didactics are offered in a variety of clinically relevant areas related to mental health relative to assessment and treatment.  Didactics are also offered to demonstrate the interaction between mental health and other forensic disciplines including child protective services, law enforcement, and the legal system.

Describe direct supervision: Supervision within our program is a student centered process.  At the onset of the training year the supervisor and extern discuss training goals that the extern hopes to achieve.  The supervisor will serve as a guide and educator in facilitating the externs growth in meeting their established goals.  The supervisor also provides education and support in clinical areas that the extern has little or no experience while also exploring how best to refine clinical skills that have been previously developed.

Learning to conduct forensic evaluations is a guided process.  As such, the extern begins the clinical training by being present in the room with the supervisor when evaluations are conducted.  The extern is encouraged to take a larger role in the evaluation after observing several evaluations conducted by the supervisor.  The extern subsequently will conduct several evaluations while the supervisor is present in the room.  After the extern and the supervisor are comfortable with the extern's clinical practice, the extern conducts evaluations independently with supervision provided subsequent to the evaluation.  A supervisor is also always available, within our center, for consultation during an evaluation.
